New Patient Forms

Prior to your first appointment, we will need to gather some important information about you.  You may visit our New Patient Forms Page on our website to complete these new patient forms, or if you would rather, you may come to your initial appointment fifteen minutes early to complete these forms in our office.  Please bring your dental insurance card with you to each appointment. 

If you have seen another dentist in the past year it will be helpful to see previous x-rays and clinical notes to compare your current condition.  You may print a record release form from the New Patient Forms Page to give to your previous dentist or visit our office at least 48 hours in advance of your appointment and we can take care of it for you.

First Visit

At your appointment, if x-rays have not been taken in the last six months a set of full mouth digital x-rays will be taken. Your dentist will perform a comprehensive dental exam at your initial dental visit.  This includes an examination of diagnostic x-rays, oral cancer screening, a gum disease evaluation, examination of tooth decay and of existing restorations.  Professional dental cleanings are also usually performed at your first visit and include removal of calculus (tartar) and plaque and teeth polishing.  If you are bringing your children and they are under the age of 18, we ask that you please wait with your child.  Important information may need to be discussed.  We do have a special waiting room where you can surf the web, watch a movie, or just relax.

If we find anything of concern during your exam, we will carefully discuss all your options so you can decide the best way to take care of your teeth.  We have intraoral cameras in each operatory as well as dental videos to help you understand any dental procedures.  You will leave our office with a treatment plan complete with an estimate so there are no surprises.  We want you to be completely comfortable with the treatment you choose.
